All your questions are indeed... questions. I actually get the impression Damon and Stefan have never been murderous psychopaths together. Considering how disgusted Damon was with Stefan in that flashback, I feel like Stefan might have gone out of his way to never have Damon see him that way again? I wouldn't be surprised if all the times Stefan fell off the wagon were times when he was estranged from Damon. (Well, except for the most recent incidents...) That would be IC for Stefan. IDK. I could be totally off the mark here.
